every year you should take them out and inspect and clean if necessary,if you get lazy and do not, it is very costly to replace your valves were talking hundreds of dollars so take it from someone who learned the hard way and i guarantee you no one will tell you different

Depends on allot of things, mileage, how rich/lean the carbs/oil pump are,allot of things impact PV cleanliness, but i would clean them once or twice a season, if you put allot of miles on 'er do it twice a year...
Oh and run a good full synthetic oil(Amsoil,Klotz, etc.)that'll help some too.

Are they supposed to just pull out of the cylinder once you remove the cable and housing? Mine move back and forth but they don't seem to want to to come out, and I don't want to damage anything.

yes sometimes they will need a tug to get 'em out as they build up with carbon on the inside of the cyl. groove where the pv slides up and down.
you can gently........ tug on 'em with pliers back and forth, they should pop out..
