Everyone talks of this being the best bang for your buck in terms of performance.. Whats being done?
I had always thought it consisted of polishing exhaust ports and smoothing rough edges within the intake and exhaust manifolds? Does that make enough of a difference?

"port/polishing"its a slang term, polishing does nothing on a 2 stroke unless the casting flash is so bad its hampering the port, its changing the heigth of the ports and width in some cases that makes the differance.

John7459- The rough edges actually help atomize the fuel mixture.

Polishing the exhaust port can never hurt ya, it can only help but is going to be a pain on the outside tripple ports. The intake is just crazy talk, you need to atomize the fuel coming in.
