Viper secondary clutch problem

I have a 02 viper with a bit of work done to it. For the second time this year I have been ridding and the slide bushing on the inner half of the secondary has come out. When it comes out and the secondary opens it jams the bushing into the jack shaft causing a whole lot of problems. Has anyone ever had this happen or have and idea why this would happen. I had both clutches cleaned in the fall. The first time this happened I blew my belt, melted plastic buttons and chewed the helix and destroyed jack shaft bearing. The second time both slide bushings came out but I caught it after it chewed the bearing but the rest of clutch was fine I can't seem to figure this out. Any help would be greatly appreciated
 

are you putting some loctitie on the bushing when putting it inthe clutch? you may need to stake the are around it slightly with a automatic center punch to hold the bushing in better. polish the shaft of the clutch with fine krokus cloth before assembling it next time.
