Slide adjustment screw coming loose?

For the past 2 weeks I have been chasing clutching issues with my 96 Vmax. Now granted, I actually had clutch issues. My roller bushings were junk, the step up shoes in the secondary were broken, and the shims needed adjustment. I finished all of that, and took the old sled for a ride and just had crappy performance.

Now before I had the clutch issues, I cleaned the carbs. I never adjusted the slides, just bowls and jets got cleaned, and put back together. I never touched the slides because it all looked good.

So after the test ride, I started looking at all the work I had done. Hand and thumb warmers, thermostat, clutch. I kept coming back to my carbs. I pulled them off, and sure enough the clutch side slide was down. Here the screw had come out of the rod, and was just flopping around. So to sync the two carbs, I opened up the other side, and that screw was also loose.

Long story short, I got it all back together and took it for a test run. It runs good. Tops out at 91 mph, but that has to do with the belt. My new one is not it yet. But has anyone else seen this issue?
 

yes at 90mph I blew my motor :( right after I got the sled, screw backed out and dropped the slide out of sync leaned down the one side when I was hauling and seized her up that was exciting to say the least, 2500miles later and they haven't moved since.
