trailer bearings

Took out a freinds boat this morning to go fishing and the bearing budding went flying as well as the wheel. Does that usually happen with bearing after awhile if they are bad? It's not my trailer so I don't know the condition of the bearings. He said he greases them regularly. Just curious is anyone on here might know the reason for the bearing buddy to just break apart and fly out like that.

did the wheel fly off also? if it did the bearings were most likely junk.
the buddy bearing is pressed ( or tapped in with a hammer ) in the hub, they can fall off, just like a grease cap would.
 
yeah everything went flying. the only thing that keep the wheel and tire on the very tip of the axle was the fender. other then that stuff was flying every where. Luckily I happened to look in the mirror at the right time or we would have lost it all! Must be the bearing was totally shot then cause there aint nothing left!! I'll have to talk him into getting the whole hub kits for both sides with new bearing buddies. I don't want to go through that again especially at 6am!! Thanks for the help!
