Piston cir clip

I was perusing another brands forum the other day and one of the things I ran across was a thread about piston cir clip failures. I have to say that in all the time I've been running and rebuilding my sleds engines,of all the failures none have been a result of a clip. Has anyone had one?
 

A friend of a friend had one, rookie rebuilder, didn't know that you had to put the clips in a certain position. Came out... Clip didn't do the damage, but the pin started to back out and destroyed the cylinder.
 
Are you saying it is a failure of the clip itself or improper installation? I have never heard of a clip failing.
 
The ends of the cir clip are not supposed to end up by the small knotch or indent on the side of the piston where the pin slides through.It's possible for the end of the cir clip to come out of the groove and end up between the piston and cylinder,causing all sorts of havoc.
