1998 vmax xtc 700 with reverse,2nd broken chaincase!!??

It was torqued, but with blue loctite and new bolt. Didn't use brake clean though. But he cleaned the threads with a thread chaser. Didn't put gear oil in it for a few days either.

I think this is your problem. If the threads of either the axle or the bolt are contaminated with oil, the loctite won't bond.

Agreed, spray the bolt and axle shaft with brake clean and blow it out with compressed air. Then use red loctite (it's not actually permanent lol) let it set up for a MINIMUM of 24 hours, longer if it's in a cold garage. If it was vibration causing it to back out you wouldn't be questioning it, that sled would shake like hell and every other bolt would be coming out as well

here are the pictures, sorry it took so long! The scratches you see around the hole are just from my husband started to rough it up for Jb weld. My husband worked on it tonight and said all he could find was that the bolt backed out. Everything else looked good.
He did use the brake clean and dried it out really well, good thing because there was a lot of oil in the hole. He also used a good amount of red Loctite. He did check the shaft for movement and it was good.
I think you guys are right about the Loctite as well! There was no sign of the blue Loctite on the threads and it was put on them. Now hopefully we can patch this outer case up long enough to make sure this works.
Rather do it that way then Pay another $95 dollars for a used one and have that possibly break!
Also wanted to thanks everyone for there help,it been appreciated!

Hmmm weird! Does yours have reverse?

I probably should have mentioned...when we purchased the sled and I said the outer case was broke then also,it was after the track was changed and We were told loctite wasn't used. Don't even know if it was torqued, but I doubt it.

I am shocked that the spring cup was riding on the cover like that and the spring still have enough pressure to keep the dog locked up on the bottom chain gear. Did someone put a washer in there that doesn't belong? A washer before the lock washer will still allow the whole mess to rotate. I would check it again just to be sure, and get a new lock washer that has a sharp edge so you know it will bite.

Everything looked correct when he just put it back together. He used the service manual with the blue yamaha on the cover. He did use a new washer as well that he got from the parts store.
